Thank you for your interest in renewing your Medical Cannabis card with Buen Salud. You received a text message as our records indicate that you have seen us in a previous year and our records indicate that it is time to renew your Medical Cannabis Card

 

We are asking patients to complete the Appointment Submission Form in order for us to schedule you an appointment.

 

After submitting your information, you will be contacted by our office via Text/Phone to schedule your Telemedicine appointment. All appointments will be done via Telemedicine. You will not need to see Dr. Smith in person.

 

After speaking with the doctor, an application will be electronically submitted to the DOH. You will get an email and all you will need to do is complete your signature and upload your ID.
